Please note some policies regarding appointments.

• You can receive a SuperBill to submit with your insurance claim if you wish.

• All affiliates are to be contacted directly regarding available appointments.  Empire Lactation does not have any direct or indirect control or responsibility for them.

• Please note protocols for Covid-19:

  •  All in person appointments are at the discretion of the provider and that decision will be made in collaboration with you.  

  • Lea remains masked at all times and asks that you do as well unless the appointment is outside.  This is as a courtesy for the safety of the next family.

• A deposit is required for most services. 

  • The credit card on file will be charged at the end of the appointment unless another arrangement is made.

  • A 50% cancelation fee will be charged if cancelation is made 12 hours or less before the appointment.

• Appointments will be arranged directly with Lea directly.
